Anagram can send an SMS or email directly to your patients with a link to fill out their information themselves. This can be a great alternative to you and your team filling out their information for them. We see this as a beneficial feature when:

  • You have a significant volume of patients and you don't have enough time to complete eligibility for each one

  • You want patients to add their own information prior to their appointments

  • Patients want to enter their own information in due to privacy concerns

Simply start by adding a new patient and providing their first name, last name, and date of birth. Next you'll be navigated to the Connect Plan page. Here you will click "Skip and Send to Patient."

Finally input either the patient's phone number OR email. Click "Send Intake Form" and a link will instantly be sent to the patient via the method you selected.

Once the patient completes their form on their end, their profile and insurance information will be listed in your patient list along with everybody else's.
