Suggested Patient Intake Information

  1. Patient's information for EHR

    1. Full name (as it appears on insurance card or driver's license)

    2. Date of birth

    3. Address

    4. Phone number

    5. Employer and employment status

  2. Ask for their vision plan information and who is the primary subscriber

    1. If they are the subscriber, collect:

      1. Employer

      2. Last 4 of SSN

      3. Member ID

    2. If they are not the subscriber, collect the primary subscriber's:

      1. Full name

      2. Date of birth

      3. Employer

      4. Last 4 of SSN

      5. Member ID
